How will I be charged for care at Desert Mountain Primary Care?

At Desert Mountain, the way you’re charged depends on your insurance coverage and the type of care you receive. If you have insurance, we’ll bill your insurance company first. You’ll then be responsible for any copay, coinsurance, or deductible based on your plan. If you don’t have insurance, we offer self-pay rates

Even if we accept your insurance carrier, you’ll want to confirm that our office is in-network for your specific plan to minimize out-of-pocket costs.

Yes, you can schedule a visit and pay our self-pay rate. If you don’t have insurance, just mention that when you call and our staff will walk you through the payment options and self-pay rates.

Copay: A set dollar amount you pay for a specific service—like a doctor’s visit, lab test, or prescription. For example, you might pay a $25 copay each time you see your primary care provider. 

Deductible: The amount you pay out of pocket each year before your insurance starts covering certain services. Once you reach your deductible, your insurance typically begins to pay a larger share of the costs. 

Coinsurance: After you meet your deductible, coinsurance is the percentage of the cost you pay for a covered service, while your insurance covers the rest. For example, you might pay 20% of the bill, and your insurance pays 80%

Yes. Desert Mountain Primary Care provides comprehensive care for a wide range of chronic conditions, including diabetes, high blood pressure, asthma, thyroid disorders, high cholesterol, and more. Our Doctors work closely with patients to create personalized treatment plans, monitor progress, and coordinate care with specialists when needed.

Yes. Sanitas Medical Group offers convenient on-site lab services at both locations. This means you can have blood work, urine tests, and other routine lab screenings done during your visit.

Please bring your medical insurance card to your appointment, along with a current list of any medications you are taking. We also ask that you arrive 10 to 15 minutes before your scheduled appointment time to allow us to update your information and complete any necessary paperwork. This helps us ensure a smooth check-in process and keeps your records accurate. 

Please contact your pharmacy and ask them to send an electronic refill request to our office. Once we receive the request, please allow 48 to 72 business hours for processing and approval. 

To have your medical records released, you’ll need to complete and sign a medical records release form. This form can be found under the Patient Information section on our website, in the Patient Forms tab.

We understand that unexpected situations can arise and you may need to cancel or reschedule your appointment. To best accommodate all our patients, we ask that you provide at least 24 hours’ notice for any cancellations or changes. 

Appointments canceled or missed without sufficient notice may be subject to a $50 no-show fee.

While we do our best to accommodate, we cannot guarantee availability, as scheduled appointments take priority. We often have same-day or next-day appointments available, so we recommend calling ahead to confirm availability and reserve your appointment time before coming in.

You can securely message your provider through our patient portal, which allows you to ask questions, request prescription refills, and receive important updates about your care. If you haven’t set up your portal yet, please contact our office, and we’ll be happy to help you get started. 

For urgent or emergency concerns, please call our office directly or seek immediate medical attention—messages through the portal are not monitored in real time.
